Understanding and improving the management of intracerebral haemorrhage: where does the TIPITCH project stand?

Spontaneous intracerebral haemorrhages (also known as haemorrhagic strokes) remain among the most severe forms of stroke, with significant consequences for patients and their families.

In response to this challenge, the French Hospital-University Research (RHU) project TIPITCH has, since 2024, brought together a large community of researchers, clinicians and partners to advance research and improve patient care.

The objective: to better understand, better treat and better support patients at every stage of their journey.

A project structured around four key pillars

TIPITCH is built on several complementary work streams, enabling action at different stages of the disease.

1. Limiting haemorrhage progression

In the early hours, one of the major challenges is to prevent the haematoma (bleeding in the brain) from worsening.

Teams are currently developing innovative miniature devices capable of temporarily modulating blood flow to limit the expansion of bleeding.
These technologies are still at an experimental stage but represent a completely new approach in the management of haemorrhagic stroke.

2. Improving haematoma evacuation

In haemorrhagic stroke, removing the haematoma is often necessary to relieve pressure on the brain.

TIPITCH researchers are developing and testing less invasive approaches, combining minimally invasive surgical techniques with clot-dissolving treatments.
A key aspect is a better understanding of clot composition, enabling treatments to be adapted to each patient.

This approach paves the way for more personalised medicine, particularly for patients on anticoagulants or at increased bleeding risk.

3. Better understanding secondary brain injury

The impact of stroke extends beyond the bleeding itself: surrounding brain tissue is also affected (inflammation, oedema, etc.).

TIPITCH is developing advanced imaging tools (MRI, PET imaging) to:

better visualise these processes
understand their consequences
identify new therapeutic targets

A clinical trial is also underway to test a drug designed to modulate inflammation and limit secondary damage.

4. Placing patients at the heart of research

This is a defining feature of the project. TIPITCH is establishing a large national cohort, which will eventually follow up to 2,000 patients over several years, in order to better understand disease progression and improve care pathways.

Patients and their relatives are also directly involved in the project through:

participation in care pathway design
contribution to research
improvement of information and support

The goal is clear: to conduct research with patients, not just for them.

A strong collective dynamic

Two years after its launch, TIPITCH is demonstrating a highly positive momentum. At the most recent annual meeting, teams highlighted:

strong collaboration between centres
innovation at the core of the project
increasing programme maturity

Interactions between researchers, clinicians and patients are helping to build a genuine collaborative research ecosystem, focused on delivering practical solutions.

Why does this research matter for patients?

Today, treatment options for haemorrhagic stroke remain limited.

TIPITCH research aims to:

improve acute-phase treatment
reduce long-term disability
adapt management to individual patient profiles
enhance long-term patient support

Although progress takes time, these advances are essential to achieving more effective and personalised treatments in the future.

TIPITCH represents one of France’s major research efforts in the field of intracerebral haemorrhage.
Thanks to a comprehensive approach — from emergency care to post-stroke follow-up, combining technological innovation and patient involvement — it is opening up concrete perspectives for improving patient management.

Highlights from the TIPITCH Annual Meeting and the Scientific Advisory Board Commission at Lille University Hospital

Monday,11th , the TIPITCH community gathered at Lille University Hospital for its annual meeting. A day rich in discussions, marked by strong engagement from the consortium and a collective commitment from across France.

A heartfelt thank you to all participants for your presence and active involvement.

Congratulations to the principal investigators for the clarity of their presentations and the outstanding scientific work shared throughout the day.

Warm thanks to the members of the Scientific Advisory Board (Prof. Lauren Sansing – Yale School of Medicine, Prof. Simon De Meyer – KU Leuven and Prof. Rustam Al-Shahi Salman – The University of Edinburgh) for their valuable feedback, insightful advice and great benevolence, which strongly contribute to the project’s momentum.

We also thank the representatives of the ANR (French National Research Agency) for their long‑term support and continued guidance.

The Wooclap interactive session launched at the beginning of the meeting captured real‑time feedback from the consortium. Your contributions highlighted strong key words — collaboration, enthusiasm, innovation, excellence. The innovation dynamic clearly emerged as one of the project’s defining features, alongside methodological robustness, strong consortium structuring and the richness of exchanges.

Once again, this meeting illustrates the strength of the TIPITCH collective and the shared energy driving research forward, in the service of patients.

TIPITCH Researchers Publish Innovative Work on Intracerebral Haematoma Evacuation

The RHU TIPITCH project is proud to highlight the work of Dr Nourou Dine A. Bankole, PhD, conducted under the supervision of Prof. Grégoire Boulouis, Prof. Marco Pasi and Dr Ilyess Zemmoura (Tours University Hospital).

Their article, recently published in the journal Neurochirurgie, presents an innovative technical note on optimising the management of intracerebral haematomas using an ultrasound-guided minimally invasive technique (HUG-MIS).

This work makes a valuable contribution by describing an approach that could improve the safety, precision and effectiveness of haematoma evacuation, a key challenge in the management of patients with haemorrhagic stroke.

Congratulations to Nourou Dine A. Bankole and his supervisors on this publication, which perfectly illustrates the innovation-driven momentum of TIPITCH and the commitment of the teams at Tours University Hospital, a key partner of the project.

Deep Intracerebral Haemorrhage: Advances and Challenges at ISC 2025

As part of RHU TIPITCH – Transforming the Prognosis of Haemorrhagic Stroke, we are committed to improving the management of haemorrhagic strokes by exploring new therapeutic approaches.

At the International Stroke Conference 2025 #ISC2025, experts shared major advances in the treatment of deep intracerebral haemorrhage—an issue at the very heart of our project.

🔹 Minimally invasive surgery: what progress is being made?
– MISTIE and MIPS, two innovative techniques for haematoma evacuation, show promising results but still require further study to demonstrate a clear functional benefit.
– Decompressive surgery, assessed in the SWITCH trial, remains a debated option with no confirmed improvement in six‑month recovery outcomes.

🔹 What levers for better medical management?
Professor Charlotte Cordonnier, lead investigator of TIPITCH, emphasised the importance of combined strategies to limit haematoma expansion, reduce cerebral oedema, and promote clot resolution. New avenues are emerging, particularly around modulation of the inflammatory response.

These advances strongly align with our mission: improving the treatment of haemorrhagic stroke and optimising care pathways.

📌 The key challenge for upcoming studies?
Identifying the patients most likely to benefit from these combined approaches and refining evaluation criteria.

The TIPITCH Project at the French National Academy of Medicine

On Tuesday 18 March, Professor Grégoire Boulouis and Professor Charlotte Cordonnier, members of the RHU TIPITCH consortium, had the honour of taking part in a session of the French National Academy of Medicine dedicated to spontaneous cerebral haemorrhages.

A prestigious moment to share scientific advances on this complex and still too often misunderstood condition.

💡 Artificial intelligence serving diagnosis and monitoring of haemorrhagic stroke

As part of his presentation on “Imaging: diagnosis and monitoring”, Professor Grégoire Boulouis (CHRU Tours) highlighted the central role of imaging and answered questions on artificial intelligence, emphasising the work underway within the TIPITCH project:

“Within the TIPITCH project, we are leading working groups in Tours to develop a large-scale imaging infrastructure and database. The aim is to gather a sufficiently large volume of data to train artificial intelligence algorithms. Structuring these data is a crucial step to enable efficient automated analysis. As of today, such a tool does not yet exist, but it is a major objective we hope to achieve.”

💡 Transforming the management of haemorrhagic stroke

Speaking on “Treatments: future therapeutic targets”, Professor Charlotte Cordonnier (CHU Lille), lead investigator of RHU TIPITCH, highlighted the major shift currently taking place in the management of haemorrhagic stroke:

“Thanks to the work undertaken, particularly within TIPITCH, we are moving away from an approach characterised by nihilism and self‑fulfilling pessimism towards a more optimistic vision based on a multidisciplinary strategy. As illustrated during this session, the management of cerebral haemorrhage now relies on close collaboration between neurologists, neuroradiologists, neurosurgeons and neuro‑intensive care specialists. Imaging and invasive treatments are integrated from the moment the patient is admitted, within a multimodal care environment that significantly improves overall management.”

💡 Transforming the prognosis of patients with intracerebral haemorrhage requires a collaborative and multidisciplinary approach. The discussions at this event reinforced the importance of innovating and rethinking the management of these severe strokes.

On Monday 20 January, Professor Charlotte Cordonnier spoke at a conference dedicated to raising awareness about stroke. The event was a real success, with strong public participation.

💡 An interactive and engaging conference
Participants were invited to answer questions through digital quizzes and animations, providing valuable insight into their level of knowledge about stroke. This interactive approach helped identify misconceptions and information gaps on this crucial topic.

🎤 Rich and insightful discussions
Professor Cordonnier highlighted recent advances in the management of different types of stroke. The hospital–university research project #TIPITCH was also presented during the session.
